UPDATED!Need Help Twin Peaks / Laura Palmer Project UPDATED!

Hello everyone,
New here was directed here by the good folks at the RPF. I am looking to do a Laura Palmer police file. Can anyone tell me where to find blank police report forms for free download/print? Or replicas of ones somebody might of done of the TPSD files they might have had on the crime? Anything like that!!! Also the correct info to fill in the file would be great, on twinpeaks.wikia.com they list Laura's DOB as 07/22/1972 and her age at death on 02/23/1989 as 16. This is obviously wrong as it is stated many times that Laura was 17. So what in the DOB would be wrong the year or month? Thanks!
